Joint Commission’s 2026 standards overhaul will reshape the framework hospitals use to guide compliance, accreditation, and performance improvement.

While the changes aim to simplify and modernize the accreditation process, the scale of restructuring is significant. Hundreds of standards have been renumbered, merged, or rewritten. For most hospitals, it represents months of work to update policies, crosswalk documentation, and retrain staff on the new structure.

What’s Changing in 2026

According to The Joint Commission’s June 30, 2025 announcement, the organization will remove more than 700 requirements from its accreditation manuals as part of its new Accreditation 360 initiative (The Joint Commission, June 2025). The goal is to streamline standards, reduce redundancy, and make compliance more transparent.

Key structural changes include:

Consolidation and reduction: More than 700 standards removed or merged, reducing the total number of hospital standards by nearly half.

New organization: Chapters such as Environment of Care and Life Safety are now merged into a single Physical Environment (PE) section.

New National Performance Goals (NPGs): Replacing the National Patient Safety Goals (NPSGs), these emphasize measurable outcomes and continuous improvement (Joint Commission, NPGs page).

Clearer mapping to CMS Conditions of Participation (CoPs): Each standard now explicitly identifies whether it is based on a federal requirement or exceeds it (HFMMagazine).

The new standards take effect January 1, 2026. Joint Commission has indicated a short grace period for hospitals to update internal documentation and crosswalks.

Why This Matters

The 2026 update is more than a renumbering exercise. It reshapes how hospitals organize, document, and demonstrate compliance. Key challenges include:

Crosswalking legacy standards to the new framework

Updating policy and procedure references

Training leaders and staff on the new numbering and expectations

Maintaining survey readiness throughout the transition

Even for hospitals with strong regulatory teams, reconciling hundreds of changes manually can take months.

Automation and AI: A Smarter Way Forward

To address the scale of change, Readiness Rounds has launched an AI-assisted Regulatory Dashboard designed to automate much of this work.

The platform:

Maintains live access to both Joint Commission and DNV standards

Automatically maps 2025 to 2026 standards as updates are released

Highlights changes and identifies affected policies and departments

Provides real-time readiness summaries for hospital leadership

For hospitals, that means no separate logins, no manual tracking, and no lag between what is published and what internal teams see. The result is continuous regulatory alignment without the clerical burden.

How You Can Prepare Today

Confirm accreditation path: Know whether your organization follows Joint Commission or DNV and centralize standards access.

Audit policy references: Identify where old numbering or chapter references appear in your policies.

Implement automated mapping early: Avoid the late-2025 scramble by deploying AI-assisted tools that auto-update as new standards release.

Educate leaders: Help department heads understand what is changing and why it matters.

Focus on continuous readiness: The new National Performance Goals emphasize sustained reliability, not just survey preparation.

Looking Ahead

The 2026 Joint Commission overhaul is ambitious. It is designed to simplify accreditation while raising the bar for performance improvement. Hospitals that adopt automation and begin aligning early will navigate the transition smoothly and stay survey-ready with less disruption.

Readiness Rounds supports hospitals nationwide in maintaining high reliability and compliance readiness.

Yet, according to national HCAHPS data, quietness consistently ranks among the lowest-scoring dimensions, with only around 60% of patients reporting satisfaction with noise levels in their hospital environment (Cambridge Sound Management, 2016).

The Technical Definition of "Too Loud"

The World Health Organization (WHO) has established guidelines recommending that noise levels in hospital wards should not surpass 30 dB L, aiming to maintain an optimal environment for patient recovery. However, actual noise levels within nursing units often significantly exceed these standards, frequently reaching or even surpassing 100 dB—comparable to the sound of a car horn—which dramatically deviates from WHO's recommended guidelines (PubMed Central, 2020).

This statistic signals a significant opportunity for hospitals to directly enhance patient experiences by addressing noise reduction proactively.

Below, we'll examine how hospital noise impacts patient outcomes and satisfaction, and explore how leveraging real-time hospital monitoring technology can effectively reduce noise, support hospital quality initiatives, and ultimately drive improvements in HCAHPS scores.

How Noise Affects Patients: More Than Just Discomfort

Excessive noise in hospital settings isn't merely an annoyance; it significantly influences patient healing and recovery. A study published in Critical Care revealed that over 58.8% of hospitalized patients voted noise as the main factor in sleep disturbance, leading to increased stress, prolonged healing times, and lower overall satisfaction (Critical Care, 2009).

In addition to sleep disruption, noise exposure in clinical settings has been linked to elevated blood pressure, heightened anxiety, and increased pain perception. According to a systematic review in the British Medical Journal (BMJ), reducing noise levels in patient care areas directly correlates with faster healing, fewer medication needs, and higher levels of patient comfort and overall satisfaction (BMJ Quality & Safety).

These physiological and emotional responses illustrate why noise management isn't merely about comfort—it's fundamentally tied to patient health and clinical outcomes.

Noise and HCAHPS Scores: The Direct Link to Reimbursement and Reputation

Patient perceptions captured through HCAHPS scores not only affect a hospital’s reputation but also directly influence reimbursement through the CMS Hospital Value-Based Purchasing (VBP) program. Under the VBP program, hospitals with lower patient satisfaction scores, particularly in categories like quietness, may face significant financial penalties.

National data continues to indicate that hospitals that improve their quietness scores typically also improve overall patient satisfaction ratings (CMS Hospital VBP Program). This presents a clear incentive for hospital administrators and quality managers to prioritize noise reduction initiatives.

Real-Time Noise Monitoring: A Proactive Approach to Noise Management

Traditional noise management strategies in hospitals typically rely on reactive approaches, responding to patient complaints or staff observations after disruptions occur. This approach is limited because it addresses noise problems after the damage—poor sleep, patient dissatisfaction, or disturbed recovery—has already happened.

In contrast, real-time noise monitoring solutions provide a proactive, data-driven alternative. Non-intrusive noise monitoring devices, such as Alertify, utilize environmental sensors strategically placed throughout patient care areas to track sound levels continuously. When noise exceeds predefined thresholds, instant alerts are sent to nursing staff or administrators, enabling immediate corrective actions.

This type of proactive noise management significantly reduces disturbances before patients complain or experience discomfort, supporting both short-term patient comfort and long-term patient satisfaction.

Practical Advantages of Real-Time Monitoring for Hospital Staff

Immediate Intervention: Real-time alerts allow staff to quickly identify and address noise issues—such as loud conversations at nursing stations, noisy medical equipment alarms, or disruptive cleaning tasks—before they negatively impact patients.

Trend Analysis and Reporting: Detailed noise logs and analytics provide hospital quality teams with insights into patterns of noise disruptions, pinpointing exactly when, where, and why noise occurs. This supports targeted interventions and continuous quality improvement strategies.

Staff Training and Accountability: Objective data removes guesswork, providing clear evidence for training staff on quiet-care protocols and reinforcing a culture of noise awareness and accountability.

Integrating Real-Time Monitoring with Readiness Rounds and Quality Audits

By using objective noise-level reports, administrators can:

Demonstrate adherence to CMS and Joint Commission environmental standards related to patient comfort and rest.

Showcase proactive steps taken to manage and improve noise levels over time.

Identify areas for further staff education or process improvement initiatives.

Hospital quality leaders—particularly Quality Managers and Chief Nursing Officers—regularly rely on digital rounding platforms such as Readiness Rounds to support compliance monitoring and prepare for accreditation surveys, helping them meet regulatory and performance standards. Real-time noise monitoring data can offer a powerful tool during these rounds, providing documented evidence of proactive environmental management.

Conclusion: Investing in Quiet for Enhanced Patient Experiences

Reducing noise isn't just an issue of patient comfort; it's a strategic imperative linked directly to patient health, clinical outcomes, satisfaction scores, and financial performance. Patient Experience Managers, Quality Managers, and Chief Nursing Officers must recognize noise control as a core component of patient-centered care.

Integrating real-time noise monitoring technologies like Alertify empowers hospital teams to effectively address the noise problem, driving measurable improvements in both patient care and HCAHPS performance. More importantly, proactive noise management underscores a hospital’s commitment to holistic, compassionate patient care—where quiet environments facilitate healing, dignity, and comfort.

As healthcare moves increasingly toward patient-centered, value-based models, hospitals that master environmental factors such as noise will gain a meaningful advantage—in patient loyalty, community reputation, and operational excellence.

Ready to Hear the Difference?

Why Patient Experience Matters: It’s the Visitors

Patient experience is a cornerstone of modern healthcare, influencing not only the well-being of individuals but also the reputation and financial health of medical institutions. Central to patient experience is the CMS HCAHPS star ratings, a vital metric that hospitals strive to excel in.

These ratings are significantly shaped by patient feedback, encompassing various aspects of their hospital visit, including their ability to navigate the facility. Visitors frequently complete post-discharge surveys, and their experiences with wayfinding directly affect their responses. A convoluted navigation process can lead to frustration, resulting in negative survey feedback that tarnishes the hospital’s ratings and reputation. In contrast, a seamless wayfinding experience can enhance patient satisfaction, foster trust, and improve overall ratings.

2. The Wayfinding Issue

The journey through a hospital can be daunting. In most cases though, the probability that you will be able to keep it current is very low. Care team

Don’t increase the number of positions shown for the care team on the board to more than a nurse, a doctor, and a nursing assistant. It would be great if you included the respiratory therapist and social worker. However, what is the likelihood that you will ever keep the additional members of the team on the communication board up-to-date on a daily basis, particularly early in the morning? Not high.

2. Ambulation/turning wheel

Ambulation and a turning wheel is great if you use it and keep it up-to-date. But what is the probability of this actually happening?

3. Patient goals

Again, a 'patient goals' section is great if you can complete it every day. Example question: "What is the most important thing we can do for you today?" This is very hard to keep up-to-date daily. Because, once it is on the board, you have to do it.

4. Hand Hygiene

Some boards and wall posters read, “Make sure we wash our hands.” REALLY?! The message to your patient is that it’s their job to make sure you do your job properly and behave safely. Is it important? Certainly. Is it the patients’ responsibility? Certainly not!!!

5. Patient ID

Some communication boards and wall posters read, “Make sure we ask your name and date of birth.” REALLY?! Is it important? Certainly. Is it the patients’ responsibility? Certainly not!!!

6. Appointment Lists

It is tempting to include today’s appointments on communication boards. If you are confident that nursing staff can keep them accurate at all times, then it is a benefit. Appointments tend to change often from initial schedule. If you have appointments on the board, but are not accurate, you are hurting the perception of care more than helping. Be thoughtful.

In order to aggregate and prioritize the data gathered by the system, administrators must know that the front-line workers entering the data are doing so accurately. (1)

Readiness Rounds recognizes the importance of vigilance in scrutinizing real-time data, both in achieving optimal outcomes for our clients and for our ability to determine and maintain our own best practices.

The following three keys to making the best use of hospital checklists come from direct, extensive experience in the field.

Within a healthcare organization, three key factors must be in place if a self-regulatory system is to function efficiently. The first two elements help ensure that the people submitting data to the system will do so accurately. The third concerns the way in which administrators handle the data once it has been collected. (2)

The 3 Keys to Effective Hospital Checklists

Key #1. It’s Not About Getting a High Score

A culture of safety must guard against the very natural human desire to “get a good grade.” Although it is important to recognize those individuals and departments that are consistently high-performing, administrators must resist creating a culture that encourages people to “spin” or alter data to present their own performance as being better than it really is. Administrators must also be willing to apply special scrutiny to departments that report at or near 100% accuracy.

Such numbers could be a red flag, suggesting that the data is idealized, not realized. The goal of a culture of safety is, of course, to score 100% on every checklist. Pragmatic administrators, however, recognize that this goal is unlikely to be reached even on a single checklist, except on very rare occasions. (3)

Key #2. Approach every observation as if it has failed

Those who interpret the data generated by checklists cannot assume that all data in the system is reliable. Such an assumption can be dangerously misleading. Even in the most highly developed culture of safety and teamwork, administrators must be willing to look carefully at the input they receive. Ideally, all aspects of data collection and aggregation should be reviewed independently by several safety team members on an ongoing basis.

Key #3. Celebrate Failures

The flip side of the first key, is that within a safety culture, it is critical that failures and shortcomings be treated sources of valuable feedback for improvement. Employees must feel safe from reprisal in reporting problems and adverse events and must know that their concerns will be taken seriously. Administrators must create an atmosphere in which failures generate cooperative problem-solving rather than punitive measures. Only in such an environment will individual workers be motivated to report data accurately, especially when the data illuminates a failure to meet expectations.

By creating a culture in which failures are seen as opportunities to improve, and the goal of 100% accuracy is seen as a process, not a requirement, organizations can minimize the motivation for individuals to idealize the data they submit. In addition, persistent scrutiny of all aspects of the data gathering process further ensures accuracy in reporting and planning and thus promotes continuous process improvement.

That’s the question Debajyoti Pati is attempting to answer. He is professor of environmental design in the College of Human Sciences, Texas Tech University, Lubbock.

Pati and his team created a mock hospital room and bathroom to study what, if any, physical elements contribute to patient falls. The team monitored patients aged 70 and older.

They found older adults frequently fell when they were performing four actions: pushing, pulling, grabbing and turning. The team also discovered that IV design is a contributor to falls because many people struggle with managing and navigating the pole while trying not to step on the IV pole’s base.

Researchers found that many of the postures involved in frequent falls have to do with navigating bathroom configuration — shutting the door, using grab bars, flushing the toilet — as getting around objects to get to the bathroom itself.

In their next phase of research, Pati’s team will focus on design solutions to increase patient safety.

Since that time our experts have partnered with healthcare professionals in a variety of roles to safeguard their facilities with a comprehensive visitor management system. Regardless of where they fall in the selection, implementation and execution stages of putting a visitor management solution in place, at one point, they had similar questions for our experts.

The four questions that our experts most often field about visitor management systems:

1. Why do we need more than an automated temperature scanning device?

We’ve seen facilities’ security go off track when they focus too narrowly on just the temperature scanning process. Temperature screening needs to be a part of the process, but it can’t be the only measure of security. A robust visitor management system includes much more than any single measure, like a temp screen, to provide the best possible safety.

2. Realistically, approximately how long does it take to put together a visitor management strategy?

Depending on your sense of urgency and resources available, the process typically takes about 30 days. In that timeframe, you can complete an 11-step evaluation, build the strategy, gain approval and get your deployment plans in place.

Note - the 11-step evaluation covers:

Scope

Entrances

Workflow

Integration

Equipment

Wellness screening

Visitor management

Notifications

PPE

Regulatory requirements

Dashboards and reporting

For more about the evaluation process, download our eBook, 11 Steps to Building an Intelligent Entrance Management Strategy in a Post Pandemic World.

3. Since facility structures are unique and visitor management crosses many departmental lines, who is “in charge” of the visitor management system at the end of the day?

Ownership of the solution can fall in different areas for different facilities. In a long-term care setting, the responsibility generally falls inside the care of the nursing structure. In acute care, security is always integral to the process, so security then manages the process. In other situations, it is patient experience, the quality department, and occasionally the admitting department. However, the key takeaway is that if you have a solid strategy in place, the organizational responsibility can land wherever it fits in your particular culture.

4. Of visitor management systems that offer several reporting options, which reports are most important?

To get a clear, top-level view without feeling buried in data, the following are generally most important:

• The trends of arrivals by employee and provider volumes, visitor volumes, vendor volumes and outpatient volumes.

• Allocation and distribution of resources, especially when it pertains to rush hour times to make sure resources are being used efficiently, and not being drained at low peak times.

• The temperature distribution over time of the population entering the facility.

Again, we know protective measures work, and vaccines will make a big difference.

However, similar to the flu, COVID-19 is unlikely to disappear and will more likely become a virus from which we must collectively and continually protect ourselves, especially the most vulnerable amongst us.

Nationally, there is polarity on whether widespread protective approaches are necessary, which can lose any ground gained in risk reduction.

Protective measures then must be an important part of healthcare’s responsibility.

Healthcare leaders must have sustainable protective measures confidently in place to not only continue fighting COVID-19, but also future infectious diseases.

But when poor patient outcomes occur, who is to blame? The short answer is “everyone.” Whether you’re a front-line nurse, a department manager, the Director of Quality, or the CEO, YOU are to blame when your hospital fails the patient.

At this point, you may be asking “How am I to blame? I wasn’t the caregiver on the floor who made the error or mistake!” The truth is that it doesn’t matter – blame takes many forms and equally touches everyone in the hospital setting, no matter what their role may be.

But have these processes and recommendations actually improved the overall occurrence of infection? Perhaps they have. At least that’s what the multitude of journal articles and newsletters lead us to believe.

Everyone working in healthcare knows that their time is strictly limited and that implementation and maintenance of a multidisciplinary, large scale project is difficult to accomplish, at best.

Could the solution be as simple as a checklist? Of course! Checklists help staff ensure that they remember to take all necessary steps and precautions when performing rote tasks, such as indwelling catheter insertion and they also make clear the required and expected steps in complex processes such as inserting central and peripheral intravenous lines, thereby reinforcing adherence to established protocols. They also provide a sense of confidence to the staff that know that they are providing not only appropriate, but safe care to their patients.

Checklists are simple and proactive tools that can be used by anyone within the organization that will provide real-time data that pinpoints straight to the area/s where performance improvement is needed the most. In addition, routine use of the checklists will help show if implemented initiatives are successful or require redesign. Used wisely, they can help an organization transition from the endless “blame game” resulting from poor outcomes towards a culture of accountability and overall improvement in patient safety and the quality of care provided.
